Our Mission:
to promote and encourage the development of well integrated domiciliary care services which model good practice as recommended in the End of Life Care Strategy (July 2008) and which span the current health and social care divide for people with palliative care needs at home.
Our Vision:
to work with commissioners, providers and service user forums to improve and enhance the current experience of service users and their families with palliative care needs by promoting the development of seamless integrated health and social care services which meet people at their point of need and enable them to die with dignity at home where that is their choice.
